    Abstract
    PT. Bosowa Berlian Motor is a service company in the automotive field that provides vehicle sales, servicing, and spare parts sales. According to data from the Center for Risk Management Studies (CRMS) in 2019, 76% of companies in Indonesia had implemented risk management. However, the implementation of risk management in improving the performance of the company's supply chain was only at 33%. The application of risk management in the supply chain at PT. Bosowa Berlian Motor has not been fully implemented. This can be seen from the various risks experienced, such as excess inventory, product damage, inadequate facility layout, machinery breakdowns, delayed customer data input, and loss of goods. These risks undoubtedly hinder the company's goals, so mitigation actions are needed to reduce the risks in the supply chain of the company. This research uses the House of Risk (HOR) model, which combines two methods, Failure Mode Effect Analysis (FMEA) and House of Quality (HOQ). The first step is to identify risk events and risk agents in each process. In HOR Phase 1, severity measurement is carried out on risk events, occurrence on risk agents, and the relationship between them, obtaining the Aggregate Relationship (ARP) value. Five risks were identified that caused 80% of the issues in inbound activities based on the Pareto principle. HOR Phase 2 identifies 10 preventive actions, and the priority of mitigation implementation should be based on implementation difficulty and cost expenditure.
