dc.description.abstractThis study focuses on the occupational accident risks within the competitive gas refilling industry, specifically regarding the refilling of 3 kg LPG gas cylinders at PT. Prima Cahaya Utama. Increased demand in this industry has led to a higher frequency of production processes, with employee safety being the primary focus in efforts to reduce accident rates, serving as a crucial indicator of the highly competitive industry's health. The analysis methods used, namely Failure Mode and Effect Analysis, along with Hazard Identification Risk Assessment and Determine Control, have proven effective in identifying risks at each stage of the refilling process. The analysis highlights that LPG gas distribution carries high risks, particularly the potential threat of gas leaks. Emphasizing gas cylinder inspections and fire safety strategies becomes essential in mitigating the identified risks. Furthermore, the study provides concrete recommendations such as regular valve and seal maintenance, comprehensive training, strict supervision, procedural enhancements, appropriate Material Handling Equipment selection, as well as health monitoring and equipment condition checks to minimize risks throughout the refilling process. The research underscores the urgency of implementing preventive measures and refining procedures to ensure safety and operational smoothness within the 3 kg LPG gas refilling industry.en_US
