This study aims to determine the risks and risk agents that can occur in the company's supply chain flow and mitigation actions that can be used to reduce the impact of the risk. The method used in this study is the House of Risk (HOR) to determine the priority of mitigation actions. In risk identification, the Supply Chain Operation Reference (SCOR) model is used as the basis for risk identification in each supply chain activity. The results showed that there were 28 risk events with 26 risk agents identified in the results of the HOR phase 1. Then using the Pareto 40/60 principle in the HOR phase 2 to determine the priority of mitigation actions. The results obtained there are 3 priority mitigation actions recommended for PT X, to reduce the possibility of risk causes in the company's supply chainen_US
