setiap perusahaan wajib untuk memberikan solusi terhadap kecelakaan yang akan terjadi dan upaya pencegahannya. Identifikasi risiko dan penilaian risiko diperlukan dalam proses tersebut, seperti melakukan pencarian penyebab suatu kecelakaan dan mengetahui akibat dari kecelakaan tersebut. Dengan mengidentifikasi setiap risiko kecelakaan maka pekerja tersebut akan mengikuti langkah-langkah prosedur dan mengenali setiap kecelakaan yang akan terjadi. Setiap perusahaan wajib untuk mengawasi dan memperhatikan para pekerja yang melakukan kegiatan di lapangan. Berdasarkan hasil observasi lapangan, para pekerja sebelum UNIVERSITAS PERTAMINA FAKULTAS TEKNOLOGI INDUSTRI TEKNIK LOGISTIK 4 melakukan pekerjaan perlu menerapkan system izin kerja aman yang berguna untuk mengetahui prosedur kegiatan dan mengetahui risiko kecelakaan yang ada. Abstract
Pertamina Hulu Energi West Madura Offshore is a company involved in oil and gas exploration and production in Indonesia which is one of the working areas in the 4 zone 11 regional area. In this area there are warehouse facilities, offices, docks, health clinics to the provision of supporting equipment for warehousing activities. In these warehouse operations, each loading and unloading area in the PT Pertamina Hulu Energi West Madura Offshore area in every operating activity that has a risk of causing work accidents. In carrying out potential hazards and risk assessment using the HIRARC (Hazard Identification, Risk Assessment and Risk Control) method. This method can identify potential risks that occur and can determine handling and mitigation. The value can be determined by a formula that is multiplying the probability by the severity level so as to get a number to determine the risk value with low, medium, and high categories. The analysis is carried out to find out what risk control strategies will be suggested or can be used by the company.1
